School Nurse Information
Hello, parents! I am Brooke Bae, RN, the school nurse at North Charleston High School. The students call me Nurse Bae.
The school packet has several forms in it. One is the School Nurse Emergency Card. This is a very important form that needs to be filled out every year, and turned in as soon as possible. The form can also be found on this page, as well as the spanish version. There will also be forms to be filled out for Dental Access (the van) that comes to the schools monthly, Telehealth forms for online visits with MUSC in the clinic will also be included but can be done online as well.
It is essential that I know about all health issues in the school as well as have parent consent to give over the counter medications that are approved by the school district. These medications are listed on the back of the emergency card and each medication you consent to needs to be checked or signed with your initials. on this page is the doctor's order form that must be filled out by a doctor for any medication that needs to be given at school from prescription medications to any over the counter medication not indicated on the Emergency form.
No medications are to be sent to school with a student (including over the counter medications). Please come to the clinic and sign in any medications as well as turn in the doctor's order form. I can not administer any medications not on the emergency card without a doctor’s order. You can find the forms here.
